Care Matching is the process of connecting our members to providers in the Modern Health network. Our Care Team is trained to match you with the best possible provider based on your unique preferences and their deep understanding of our provider network. Connecting you to care is our top priority, so we do our best to match members within 2 business days with a provider we believe is the best fit.
Our Care Team’s Process
- We receive your care request with your preferred focus areas, location, and availability.
- Once we see your request, our Care Team will immediately notify you that your care request was received.
- We take each of your unique preferences into consideration and identify the best-fitting provider.
- Once identified, we connect you to your provider via a separate email where you can coordinate an initial call or session.
Our Care Team will ensure the provider you’ve been matched with has expertise in the areas you’ve prioritized. If at any time and for any reason you feel like it isn’t a great fit, our Care Team would be happy to work with you to ensure a better match!
