How Can I Delete My Data?

By requesting a data deletion, you are confirming that you would like Modern Health to delete your account and the personal information associated with your account. Please note, by deleting your account you won’t be able to reset your password and access your account moving forward.
To request a data deletion, you can follow these steps:
- Fill out this form to start the process of deleting your account and data. Please use the email associated with your Modern Health account.
- Once you click Submit Request, copy the 4-digit code.
- You will receive an email to the email you used to verify your identity. Please note the link to verify will expire in 1 day.
- Once you click the Confirm Email link in the email, enter the 4-digit code.
- Your request will be submitted and you will receive an email receipt that your request is being processed. You can expect your account and data to be deleted within 30 days. Optional: You may choose to create an account in our DSR Request Center to track the status of and manage your request.
- Once your request is completed, you will receive an email confirming that your data and account have been deleted.
Note that if you are still eligible for the Modern Health benefit through your employer, Modern Health will continue to receive your eligibility information contained within their eligible persons file. If you would like to be removed from future files sent to Modern Health please contact your employer.

How Can I Unsubscribe from Modern Health Notifications?
You can subscribe and unsubscribe to push notifications on the mobile app under Settings > Notifications. You can unsubscribe from certain email notifications by selecting Unsubscribe at the bottom of the email you have received. Please note you cannot unsubscribe from essential account related emails such as upcoming appointments or Messaging reminders.
