By switching to paying out of pocket, overall pricing for continued care is at your provider's discretion. While we encourage our providers to offer great pricing for Modern Health members, overall pricing is at their discretion.

You will need to retrieve your provider(s) contact information to plan your next steps as a client outside of your employer's benefit plan. Based on your communication preferences with your Care Team, you can directly email or use the Messages feature to contact your provider(s) for their contact information. Also, you can contact our team at help@joinmodernhealth.com to retrieve this information.
- Your provider may refer you to an alternative provider who takes your health insurance. Also, we encourage you to contact your health insurance to see what other care options are available to you.
